Optical Metrology Questionnaire

In the interest of keeping the testing process as efficient and cost-effective as possible, we would like to know more about the optical system including specifications, additional requirements, etc. Please answer the following questions to the best of your knowledge. If a lens prescription or list of specifications can be supplied in addition, please send them along with this questionnaire.

Please include such information as the orientation of the lens (image/object), as well as any gate marks on the lens, which direction they should face (e.g., a mark on the barrel indicating it should be oriented in the sagittal/tangential plane), any additional optical elements that need to be placed in the system (e.g., glass in the object plane for a copier/scanner lens), source considerations, special fixture requirements, etc.
